Trying to Create a New Invoice Pro-Forma

I want to create a new Invoice proforma showing my company details & logo. How do i go about doing this?

The fact that it is a proforma is not relevant.
The layout for an invoice is the same regardless of if it is a proforma, credit note or invoice.
The words "Invoice, Credit Note or Proforma" are a variable on the layout.
The best place to start is editing and existing layout. Ot subcontracting the job out to someone like me...
